New Patient Information
Scheduling Your First Visit
When you call to schedule your first visit we will ask the following:
- Basic demographics
- Email and cell phone number – we will send you text message reminders via your cell phone.
- Insurance information if any
- Type of treatment – Chiropractic or Acupuncture
- Provider preference
We ask that you arrive at least 15 minutes early to complete your paperwork. Please bring a copy of your insurance card to your first visit, so we can scan it for our records.
